About This Grant
PROJECT SUMMARY. Mycobacterium tuberculosis (Mtb) latently infects about one-fourth of the world’s population, causing pulmonary tuberculosis (TB) in ~10.8 million people and resulting in ~1.25 million deaths in 2023. TB has returned to being the leading cause of death globally from a single infectious agent. The currently available TB vaccine, Mycobacterium bovis Bacille Calmette-Guérin (BCG), shows variable efficacy. In the absence of clear correlates of protection for TB vaccines, it is imperative that we develop novel strategies that target broader T cell responses against TB. Our research has shown that T helper type 17 (Th17) cells producing interleukin-17 (IL-17) is an important effector cell, mediating vaccine-induced protection against Mtb. The respiratory tract is the natural route of Mtb infection; thus, mucosal vaccination is known to induce superior protection against Mtb challenge, when compared with parenteral routes of vaccination. Enhanced protection by mucosal vaccination is thought to be a result of preferential trafficking of antigen (Ag)-specific, vaccine-induced T cells back to the site of vaccination, possibly localizing within the airway lumen. Nanoemulsions (NE) are oil-in-water emulsions formulated with antigens. A newly developed NE adjuvant platform (NE01) was found to be safe and well-tolerated in human volunteers in two Phase I trials when used as a flu vaccine, eliciting both systemic and mucosal immunity following intranasal (I.N.) vaccination. Furthermore, NE vaccine delivered with antigens in mice by I.N. route induced Th17 responses. When delivered with immunodominant Mtb antigens (Ag85B and ESAT6), NETB-EA vaccine can confer Mtb control, and decreased TB disease in mice and nonhuman primates. The work proposed here will allow preclinical development and advancement of the NETB-EA vaccine towards licensure as a first-of-kind Th17-inducing mucosal TB vaccine for clinical trial readiness by achieving the following Specific Aims. Specific Aim 1. Selecting a Production System for Mtb antigens. Specific Aim 2. Development of a Scalable Process to Purify Preclinical Grade Mtb antigens for NETB-EA Vaccine. Specific Aim 3. Preclinical Efficacy, Durability, MoA and CoP analyses of NETB- EA vaccine in BCG-vaccinated mice. Specific Aim 4. Investigational New Drug (IND) Application Enabling Safety Studies of NETB-EA in Rabbits. Successful completion of these aims will establish the NETB-EA vaccine as a well-characterized, Th17-inducing mucosal TB vaccine ready for advanced preclinical testing and future clinical development.
